摘 要

目的:研究VSD负压引流联合纳米银敷料在34期压力性损伤感染创面中的应用效果,为34期压力性损伤感染创面的患者制定合理的护理方案、评价护理效果提供依据。方法:选取20193~20213月一年时间中,在我院就诊的34期压力性损伤患者320例,并将其平均分为两组,即观察组与对照组。前者采用的治疗方式为VSD负压引流技术联合纳米银离子敷料;后者使用VSD负压引流技术进行治疗。两组进行数据对比,对比的因素有临床治疗效果、换药次数与费用、创面愈合与肉芽组织生长时间。结果:观察组上述因素都明显低于对照组(P < 0.05),其临床治疗有效率(86.88%)显著高于对照组(59.38%,P < 0.05)。结论:采用VSD负压引流技术联合银离子技术对34期压力性损伤患者进行治疗,可以减少换药次数以此来节省费用,同时患者所承受的疼痛也有很大程度降低,并且创面愈合速度更快,综上所述,其临床应用价值较高,值得进行推广。

Comparative Application Study of VSD Negative Pressure Drainage Combined with Nano-Silver Dressing in the Treatment of Stage 3 and 4 Pressure Injuries

Abstract

Objective: To study the application effect of VSD negative pressure drainage combined with nano-silver dressing in stage 3 and 4 pressure injury infected wounds, so as to provide basis for formulating reasonable nursing plan and evaluating nursing effect for patients with stage 3 and 4 pressure injury infected wounds. Methods: A total of 320 patients with stage 3 and 4 stress injury admitted to our hospital from March 2019 to March 2021 were randomly divided into observation group and control group, with 160 cases in each group. The control group was treated with VSD negative pressure drainage, and the observation group was treated with VSD negative pressure drainage combined with nano-silver ion dressing. Pain degree, dressing change times, dressing change cost, wound healing time, granulation tissue growth time and clinical treatment effect were compared between the two groups. Results: The number of dressing change, cost of dressing change, wound healing time, pain degree and granulation tissue growth time in the observation group were significantly lower than those in the control group (P < 0.05). The clinical effective rate in the observation group (86.88%) was significantly higher than that in the control group (59.38%, P < 0.05). Conclusion: We use VSD negative pressure drainage technology to combine silver ion technology to treat patients with stress injuries 3 and 4, which can reduce the number of changes to save costs large extent, and the wound healing is faster, at the same time, the pain of the patient has been greatly reduced, and the wound healing is faster. In summary, its clinical application is worth promoting.

Abstract

Objective: To explore the treatment and nursing characteristics for pressure injury on the inside of the thigh caused by forced posture in patients with cirrhosis and diabetes mellitus.

Methods: A patient with cirrhosis and diabetes mellitus complicated with abscess of right inner thigh pressure injury was selected as the study object, who was admitted to Xuzhou Medical University Hospital on June 29, 2019. The analysis of the clinical diagnosis, treatment and nursing process were applied to explore the prognosis of the patient with cirrhosis and diabetes mellitus complicated with deep abscess of the thigh.

Results: The traditional treatment including incision and drainage was not allowed, as the general treatment was not effective for the patients with cirrhosis and diabetes mellitus complicated with deep abscess in the inner thigh. After 8 days of negative pressure manual dressing change combined with mupirocin ointment treatment, the local skin condition returned to normal.

Conclusion: After literature search, it was found that the above-mentioned cirrhosis combined with diabetes and deep abscess cases in the inner thigh were rare clinically. When the traditional treatment of incision and drainage is limited, negative pressure manual dressing change combined with antibiotics is an effective method, which is worthy of clinical application and promotion.

摘 要

随着人们年龄的增长,不可避免地出现机体各项生理功能下降,慢性病发生率上升,逐渐走向衰弱,直至生命终结。机体衰弱和糖尿病通常同时出现在某些老年人,且有部分共同的发病机制和危险因素。这篇文章旨在综述近年来对两种病理情况的新认识,描述它们的相互关系研究和治疗策略进展。

Abstract

With the growth of people’s age, it is inevitable that the physiological function of the body declines,and the incidence of chronic diseases increases, and gradually weakened, until the end of life. Frailty and diabetes mellitus usually occur simultaneously in some elderly people, and share some common pathogenesis and risk factors. This article aims to review new insights into two pathological conditions in recent years and describe their interrelationship research and advances in therapeutic strategies.

糖尿病肾病是糖尿病重要的微血管并发症,引起持续进展的慢性肾脏损害,导致肾功能衰竭。糖尿病肾病作为慢性肾病患者主要病因及死亡原因,极大地加重了全球健康及经济负担。糖尿病肾病发病机制较为复杂,现有治疗方案效果有限,亟待进一步探索发病机制,以更好地预防、治疗、管理糖尿病肾病。长链非编码RNA参与了许多生理过程,如细胞周期调节、细胞凋亡和存活、肿瘤迁移和代谢等。某些LncRNAs在糖尿病肾病发病机制中发挥重要作用。本文综述了参与糖尿病肾病发病机制、作为生物标志物或在临床应用中作为治疗靶点的LncRNAs

Abstract

      Diabetic nephropathy (DN) is a major microvascular complication of diabetes mellitus, leading to gradual progress in chronic deterioration of renal function, eventually resulting in the occurrence of kidney failure. DN is the major cause of the high morbidity and mortality of chronic kidney disease (CKD), which significantly aggravates the healthy and economic burden globally. Therefore, it is important to explore the underlying mechanism for prevention and management of DN due to the complicated pathogenesis and limited therapy. Long noncoding RNA (LncRNA), a novel target, is involved in many physiological processes, such as cell cycle regulation, apoptosis, cell survival, tumor migration and metabolism. Furthermore, evidence suggests that LncRNAs play a vital role in DN pathogenesis. This review summarizes LncRNAs, which are involved in the pathogenesis of DN, and used as biomarkers or therapeutic targets of DN in clinical practice.
